Eye Checkup

An eye exam involves a series of tests to evaluate your vision and check for eye diseases. Your eye doctor is likely to use various instruments, shine bright lights at your eyes and request that you look through an array of lenses. Each test during an eye exam evaluates a different aspect of your vision or eye health.

Lasik Laser

LASIK, which stands for laser in-situ keratomileusis, is a popular surgery that can correct vision in people who are nearsighted or farsighted, or who have astigmatism.

WHY IS LASIK DONE?

When light doesn’t focus on your retina the way it should, your vision is blurry. Doctors call this a refractive error. The basic types include:

  • Near-sightedness (myopia). You see things clearly when they’re close to you, but things farther away are blurry.
  • Farsightedness (hyperopia). You see faraway things more clearly, but closer things are blurry.
  • Astigmatism. This can make everything blurry because of how your eye is shaped.

Standard Lasik

Standard lasik or conventional Lasik is a process to reshape the cornea using an advanced Laser to correct the refractive error of the eye. The amount of reshaping done to the cornea is determined by some diagnostic tests before the procedure.

Standard laser vision correction surgery uses the prescription in a pair of eyeglasses as the basis to correct vision. It does an excellent job of reducing or eliminating the need to wear glasses or contact lenses. Standard Lasik surgery cannot correct spherical aberration.

BENEFITS OF STANDARD LASIK:
  • Clear Vision without Glasses or Contact Lenses
  • Corneal Tissue Saving
  • Maintain Natural Shape of Cornea
  • Increased quality of Vision

Implantable Contact Lens (ICL)

ICL surgery is a vision correction procedure that is FDA approved to treat nearsightedness (myopia) and astigmatism in the United States. This is an artificial lens that is surgically implanted inside the eye. It is carefully placed between the iris, which is the colored part of the eye, and the natural lens.

ICL surgery is suitable for those who have –
  • Shortsightedness from -0.50 to -20.00
  • Farsightedness from +0.50 to +10.00
  • Astigmatism from 0.50 to 6.00
  • Aged between 21 and 60 years old

The ICL is used for vision correction for shortsightedness (myopia) from -0.50 to -20.00 D and farsightedness (Hyperopia) from +0.50D to +10.00D. Toric ICL implants correct astigmatism from 0.5D to 6.00D.The ICL can be used as an alternative to Lasik Laser eye surgery and has the advantage of being “Additive” and can therefore be reversed. The levels that can safely be corrected are much higher than Laser eye surgery.

ICL surgery is a good option for those with Dry eye, Keratoconus, those who have thin corneas or abnormally shaped corneas including those who have had corneal transplants.

Refractive Lens Exchange

A refractive lens exchange is a safe and effective procedure for many patients who have been told that they are not a good candidate for LASIK or PRK. With LASER eye surgery, a microscopic amount of tissue is removed from the cornea to help the light focus properly on a patient’s retina. With an RLE procedure, a patient’s cornea remains intact. Instead of removing tissue from the cornea, a patient’s natural crystalline lens, which sits behind the colored part of the eye known as the iris, is removed and replaced with a biocompatible manmade lens. This lens will have an exact prescription unique to the patient’s refractive error. This procedure is very similar to cataract surgery, which many people will need later on in life.

Refractive Lens Exchange is for those who:
  • Any patient who is over age 40-5o who is nearsighted, farsighted, or has astigmatism.
  • Want to reduce or eliminate their dependence on glasses or contact lenses
  • Have early clouding of the lens (dysfunctional lens syndrome)-which is uniform after age 50
  • Anyone who is nearsighted, farsighted or has astigmatism who has early cataracts (clouding of the lens)
  • Want to treat presbyopia with a multifocal implant
  • Are looking for an alternative to LASIK

2. Phaco Surgery and Its Types

In Phaco technique, a small incision is placed in the eye, and a hollow Phaco needle is introduced through the opening. On giving energy through the tip of the Phaco needle, the cataract lens gets dissolved and is sucked out through this needle. An artificial lens called a foldable Intra-ocular lens is now injected through this small opening. Once inside the eye, it opens up and serves to replace the original lens. Since the eye-opening is very small and designed specially, it self-seals, and no sutures are required. Hence this procedure is also popularly known as “Stitchless” cataract surgery.

Regular Phaco Cataract Surgery It is a surgical technique wherein the cataract is emulsified using ultrasonic energy. The incision size is about 3.0mm.

Micro Phaco Cataract Surgery This is the technique where the cataractous lens is dissolved and aspirated through a 2.2mm micro incision and a special super soft Intra Ocular Lens is introduced through the same incision. The incision in the Microphaco technique is much smaller than the incision in a Regular Phaco technique. Micro phaco is designed to speed the recovery of vision, improve visual outcomes and reduce the restriction on your activities after the surgery. The machines used for Microphaco is either the Centurian machine from Alcon (USA) or the Stellaris machine from Bausch & Lomb (USA), both of which are in use at the Bathinda Lasik Laser Centre. Features of Micro-Phaco:

Micro-incision
  • No stitch
  • Walk in- Walk out Surgery
  • Faster recovery
  • Reduced need for glasses

3. Microincision Surgery – All that You Wanted to Know

The definitive treatment of cataract is primarily surgical. Over the years, the surgical technique has undergone a revolution regarding both, technique and patient outcomes. Today, cataract surgery is both safe and effective. In an attempt to further improve patient comfort, the advancement in technology means faster recovery and better visual rehabilitation.
It began with small-incision cataract surgery with an incision less than 4mm in size, as against a 12-14 mm incision in conventional cataract surgery (called extracapsular cataract extraction). The introduction of phacoemulsification changed cataract surgery forever, heralding the onset of sophisticated mechanization. The most significant advancement in this regard are, one, a decrease in the size of the incision, and two, the use of premium IOLs. The latest in the former is the development of the surgical technique, Microincision Cataract Surgery (MICS); where incision size is less than 2mm in size.

Types of Premium IOLsWhich is the best IOL for you?

In medicine, like life, there is no single best answer. Your doctor will discuss with you the potential benefits of each of these lenses, and help you select the one best suited for your lifestyle and visual needs, as well as your expectations from the cataract surgery.

Lifestyle and priorities: In case you have a very active lifestyle that involves a lot of precise focusing outdoors, like golf, you may compromise your near vision but not your distance vision. If you travel a lot, you might prefer to not be dependent on glasses at all, proceeding a bit of precision for both near and distance vision. On the other hand, if you enjoy sewing or embroidery, you will want perfect near vision.

Pre-existing diseases: In case of certain eye diseases like advanced glaucoma, corneal disorders or age-related macular degeneration, you may not be a good candidate for specific lenses, like the multifocal lens. Your eye doctor will perform a comprehensive eye exam and discuss its results and implications with you, to help you choose the best possible lens for your eye.

Costs: These lenses are more expensive than traditional IOLs, adding an incremental value to the cost of cataract surgery. The premium lenses come at a price that is usually not covered by insurance companies.

Dr. Yuvraj Markan at Bathinda Lasik Laser Centre is well versed in the use of premium IOLs following cataract surgery in Punjab region and have had the experience of implanting thousands of these lenses. Dr. Yuvraj Markan will be happy to discuss all the options available and help you choose the best choice for your eye health, and visual needs.

Retina

The retina is like the film of the camera which sends the image to the brain for processing. A damaged retina can lead to significant visual disturbances many of which may become permanent if not treated in time. The retinal diseases are some of the most common causes of a permanent loss of vision in many patients, if not treated on time.

Problems Associated with Retina

There are four main types of disorders which are related to the retina:

  • Diabetic Retinopathy – It is one of the most common issues associated with people having diabetes. In Diabetic Retinopathy disease, the high blood sugar levels cause damage to the blood vessels of the retina and damage it. In the advanced stages, it can lead to blindness.
  • Retinopathy of Prematurity – RoP is mainly caused due to the retinal blood vessels developing abnormally and affects prematurely born babies. It can also lead to retinal detachment and blindness.
  • Age-Related Macular Degeneration – The damage of Macula can cause AMD which can lead to a permanent vision loss.
  • Retinal Detachment – It is mainly the detachment of the retina from the layer underneath. If it is not taken care of properly, there is a chance of a permanent vision loss.

Fluorescein Angiography is one of the most common tests that are done to identify the problems which can cause permanent blindness due to the disorders of the Retina.
Based on the results, the patients are either suggested for Anti-VEGF Therapy or Photodynamic Therapy depending upon their condition.

Galucoma

Glaucoma is the most serious eyesight threatening condition. It usually manifests as a painless, gradual loss of vision. The lost vision can never be recovered. However, medical or surgical treatment can prevent or retard further loss of vision.

Do Not Confuse Glaucoma with a Cataract

Many times it can be confused with a cataract which also manifests as a painless, gradual loss of vision. The difference is that in the case of cataract, the loss of vision is fully recoverable using a simple surgery called Phaco.

Our eyes contain a clear fluid called aqueous humor, which is continuously produced in the eye to bath and nourish the structures inside it. The fluid normally drains out of the eye through drainage canals in a fine meshwork located around the edge of the iris (the colored part of the eye that surrounds the pupil). In people with glaucoma, the fluid fails to drain due to some defect and thus increases the pressure inside the eyes called raised Intraocular Pressure (IOP) (or Tension)

Squint

Squint or strabismus is a disorder in which the eyes are not aligned in the same direction. It is more commonly seen in children and may be associated with amblyopia or lazy eye.

WHAT CAUSES SQUINT

The reasons for squint in children and adults are different.

  • A latent childhood squint may become prominent with eye strain in the later stage of life.
  • A loss of vision in one eye can result in its outward deviation.
  • New squint that develops in an adult can be due to disorders of thyroid eye disease, myasthenia gravis, trauma, stroke or tumors
WHAT ARE THE SIGNS AND SYMPTOMS OF SQUINT?

The most noticeable sign of squint, of course, is the misalignment of eyes, which can be noted by the patient, as well as by the observer. Associated symptoms of squint include:

  • Eye fatigue and tiredness around the eyes
  • Double vision which can be very annoying, and often leads to intractable headaches
  • Overlapped or blurred images causing confusion
  • Difficulty in reading and near work
  • Loss of depth perception resulting in stumbling and past pointing

Pediatric Ophthalmology

Paediatric Ophthalmology has now been recognised as a distinct speciality in ophthlamology by Government of India, vision 2020 and various leading eye care organisation. Childhood blindness refers to a group of diseases and conditions occurring in childhood or early adolescence, which, if left untreated, result in blindness or severe visual impairment that are likely to be untreatable later in life.

SYMPTOMS
  • Watery eyes
  • Dry Eyes
  • Drooping eyelids
  • Protruding eyes
  • Lid tumors
  • Skin wrinkles near eyes
  • Fractures of the orbit
  • Disfigurement of the eyelids
